Tours & Site Visits
We offer tours of PASS facilities for interested individuals and families. These tours are usually arranged by your Personal Agent [at your Support Services Brokerage] or Case Manager [at the county in which you live]. If you do not have a Case Manager or Personal Agent you should get one – which one you need will depend on your individual situation. Generally, the county is the place to start that process. If you are not eligible for a PA or CM and would like to visit anyway we would be happy to arrange to meet you. Just call us at (503) 262-9322 and ask for Evvalee or Amy. Or email us at: passpdx@gmail.com. They can talk with you about PASS and its services (Day Program, Community Activity Program [PASS-2], and Respite Care) as well as set up a tour at a mutually convenient time. PASS services are designed around support for the individual and family . . . Our 3-tier fee structure is designed to match client needs with staffing resources while maintaining a higher than average staff to client ratio - one of the hallmarks of the PASS 'model.' In general we do not accept new clients with high medical or behavioral needs; however, each request for services will be evaluated on its own merits by our Management Team.
